Request handling
The embedded quote form is the existing Simple Water Service form hosted by LeadConnector. Information submitted through it is handled by the provider and its form service under their applicable notices. Read the form’s displayed consent language before submission. This site does not store assessment answers or submit them to a server. The educational assessment runs in the browser and is separate from the contact form.
External services and links
Opening the quote form contacts its hosting service, which may process connection information as part of displaying the form. External links to utilities, the health department and the provider take you to those sites and their policies. No new analytics or call-tracking script is installed in this review build. Any production tracking must be configured and reviewed before launch, with this notice updated to reflect the actual implementation.
Contact and limitations
For questions about a request sent through the provider form, contact Simple Water Service at (561) 309-6270. Do not submit sensitive medical information through an ordinary quote request. The educational content does not diagnose water safety or replace official advisories, laboratory results or an inspection. A form submission requests contact and does not create a confirmed appointment or installation contract.
